Clinical Pharmacy Specialist - Oncology

The Clinical Pharmacy Specialist (CPS) provides pharmaceutical care services through direct interaction with patients and providers and functions at the highest level of clinical practice working independently under their individualized scope of practice. The Clinical Pharmacy Specialist serves as a non-physician provider who functions to initiate, modify or discontinue medication therapy and as a consultant for intensive medication therapy management services. These activities require a great degree of skill in patient monitoring, drug therapy review and assessment of patient outcomes. The CPS's medication therapy management services include, but are not limited to:
designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medications. The CPS performs physical assessment, orders and interprets laboratory and other diagnostic tests utilized to monitor and assess outcomes of drug therapy. This contributes to the achievement of positive patient centric outcomes through direct and indirect interactions with patients, providers and interdisciplinary teams in assigned clinical areas. As a Clinical Pharmacy Specialist in Oncology, services shall include, but are not limited to the following:
unit dose medications, parenteral products, cytotoxic chemotherapy, enteral products, supply items, etc., for inpatient and/or outpatient services, monitor patient drug therapies, and provide drug information. The oncology clinical pharmacy specialist is involved with the evaluation of non-formulary consults in accordance with VA criteria for use and when appropriate offers medication alternatives for non-formulary medication use. The Clinical Pharmacy Specialist will support the Oncology team in the education of patients on proper medication use and dosage, untoward affects, and if necessary how to interface and use outpatient pharmacy prescription services. Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Experience:
PGY-2 Pharmarcy Residency Specializing in Oncology/Hematology Grade Determinations:
GS-13 (a) Experience. Must have 1 year of experience equivalent to the next lower grade level. (b) Assignments. Candidates at this grade level are to be in one of the assignments listed below. For all assignments above the full performance level, the higher level duties must consist of significant scope, administrative independence, complexity (difficulty) and range of variety as described in this standard at the specified grade level and be performed by the incumbent at least 25% of the time. 1. Clinical Pharmacy Specialist. The clinical pharmacy specialist (CPS) functions at the highest level of clinical practice, works independently under their scope of practice as defined by the individual medical center to directly care for patients. A CPS plays a defined role in budgetary execution and serves as a mid-level provider who functions to initiate, modify or discontinue medication therapy and as a consultant for intensive medication therapy management services. This includes, but is not limited to, the following:
designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments; helping achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas; performing physical assessments; and ordering laboratory and other tests to help determine efficacy and toxicity of medication therapy. Pharmacists assigned to this position must demonstrate the following KSAs:
a. Ability to communicate orally and in writing to persuade and influence clinical and management decisions. b. Expert understanding of regulatory and quality standards for their program area. c. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. d. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. e. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. References:

A pre-placement physical examination is required for all full-time, part-time and intermittent employees identified by VA Directive and Handbook 5019. The pre-placement physical examination shall be completed prior to appointment to determine the physical and mental fitness of candidates for appointment in VA. The examination will be concerned with the mental and physical ability of the applicant to satisfactorily perform the duties of the proposed assignment and will include requisite laboratory and other screening as required by Federal regulatory agencies. A pre-placement Drug Screening is required and position is subject to random screenings thereafter.
