$45,000- $48,000 (exempt) Catholic Charities Senior Communities develops and operates affordable, supportive communities for older adults with a resolve to nurture a spirit of purpose, wellness, and harmony among both our residents and colleagues.
We offer 24 locations in Anne Arundel, Baltimore, Garrett County and Harford Counties and Baltimore City.
Catholic Charities of Baltimore, Senior Communities is currently seeking a Service Coordinator who educates, assists and advises residents and families about the resources and services to support living in the apartment setting, encouraging and coaching residents to be proactive self-advocates in meeting their social, psychological, and physical needs.
The work schedule is Monday- Friday, 8:
00am -4:
00pm.
JOB DUTIES &
Responsibilities:
Monitors resident capacity and responds to referrals regarding adequate access to nutrition, transportation, medical care, mental health services and counseling.
Provides case management for residents as needed, including for those lacking any family or other advocate.
Develops relationships and acts as a liaison between residents and community agencies, service providers, and advocates and meets with these entities as needed.
Negotiates on behalf of residents for adequate, timely and cost effective provision of services and access to needed resources.
Responds to difficult situations including issues of self-neglect, cognitive-impairment, chronic mental health concerns and crises.
Supports positive communication in resident-resident, resident-family, and resident-management conflicts.
EDUCATION & EXPERIENCE REQUIREMENTS:
Bachelor's degree and three years' experience in Human Services, preferably in the field of aging or an equivalent combination of education and experience.
Must be able to work effectively with culturally diverse populations.
Conduct effective one-on-one interactions with residents; maintain confidentiality.
Write/print legibly; respond to resident/family needs professionally; work independently and as a productive member of a team.
Develop a thorough working knowledge of community resources; comply with government and funding regulations and agency policies and procedures.
Meet deadlines; organize and prioritize work; and have reliable transportation to and from assignments.
Must have a current MD driver's license with no more than three points and reliable transportation.
PHYSICAL REQUIREMENTS & WORK ENIVIRONMENT:
The Service Coordinator reports directly to the Community Manager and works self-directed 95% of the time.
The incumbent is expected to perform the duties of the job following established procedures and in accordance with the Agency policies and to exercise initiative and independent judgment and to seek advice and assistance for unusual/difficult cases.
The incumbent will have on-going interaction with housing management, residents, families and community service providers.
This position requires intermittent sitting, standing, bending and lifting 15-30 lbs.
; the ability to move freely throughout the building and resident apartments including climbing stairs; to have visual and hearing acuity and clear speech sufficient to perform the job effectively and dexterity sufficient enough to produce case notes and enter information into required software.

The check will bounce and the victim is left responsible.
